The First Visit

Please provide the following information at the time of your first visit:

  • If you have dental insurance, bring your insurance card and necessary completed forms.
  • Your referral slip and any X-rays (if applicable).
  • A list of medications you are currently taking.


IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at all appointments.


Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)


Medical History

The initial visit involves a complete review of your medical history. When you share your medical history, be sure to provide up-to-date information on your health. Inform our office if you have experienced a recent hospitalization or surgery, or if you have been ill. Also tell us the names, doses, and frequency of any medications you are taking — whether prescription or over-the-counter products — and the name of your physician. Always inform us of any changes in your health or medications. This information will help us select the safest and most effective method of treatment for you.


X-rays are a very important part of the comprehensive examination. They allow us to see areas of your teeth/mouth that we cannot see visually, including decay between your teeth or position of wisdom teeth. If your previous dentist has taken x-rays, you may request that they be sent to our office or pick them up and bring them to your appointment. Additional radiographs may be necessary and will be taken at our office using our digital radiography equipment technology.


At this first visit, we will provide you with the necessary procedure you need and deserve, whether it is a routine cleaning using new ultrasonic scaling or emergency dental treatment with effective pain management.
